Key Responsibilities
Ensures continuity of care for newly enrolled members
Develops and monitors the Person Centered Service Plan while
managing a caseload
Facilitates communication and care coordination among an
interdisciplinary team
Required Qualifications
Experience in care management or related field
Ability to assess member needs and manage care services
Familiarity with regulatory policies and documentation
requirements
Experience working with interdisciplinary teams
Proficiency in technology-based documentation solutions
