Job Description:
A company is looking for a Social Worker to join their Case
Management team.
Key Responsibilities
Coordinate care across various healthcare settings while
maintaining confidentiality
Develop unique care plans for members facing health challenges and
guide them through the healthcare system
Assess Social Determinants of Health needs and educate members on
community support resources
Required Qualifications
2+ years of medical social work experience in a healthcare facility
setting
1+ years of experience in care coordination and navigation
Master's in Social Work (MSW) from a nationally accredited
school
Licensed Master Social Worker or Licensed Clinical Social Worker
(required for specific states)
Bilingual in Spanish and English (preferred)
