A company is looking for a Care Management Support Coordinator

Key Responsibilities
Provides outreach to members via phone to support care plan next
steps and connect them to resources
Serves as the front-line support for member and provider inquiries
and maintains non-clinical member records
Supports member onboarding and administrative duties, ensuring
compliance with state and regulatory requirements
Required Qualifications
High School diploma or GED
1 - 2 years of related experience
Knowledge of health plan activities and resources
Ability to document and maintain records in accordance with
policies
Familiarity with Social Determinants of Health (SDOH) needs
